Integrated Community Supports

Grandma’s Place is committed to supporting individuals in developing independent living skills in their own home by providing Integrated Community Supports. We have relationships with property managers at four locations in Crystal and Minneapolis. We provide services for high-functioning individuals with:

• Intellectual disabilities
• Chronic/Persistent mental illnesses
• Brain injuries
• People who may have faced substance abuse/misuse barriers in the past
