Thank you for your interest in Grandma's Place. Grandma's Place is a premier provider of services, working with individuals to become their best. We have clients who have been receiving services from Grandma's Place for as long as 15 years! Our goal is to make sure that Grandma's Place is a good fit for the client and the client will be a good fit for us.
Grandma's Place Criteria
Criteria:
All criteria is verified through documentation, background studies, rental verification and interviews with providers, family and/or friends.
Waivered Service Provider:
- Accepting CADI, BI or DD waivers
Desire to want services:
- Desire and need to receive 20 to 40 hours of services per week
Individuals served must not exhibit recent history of behaviors that endanger self, other individual
- No recurrent violent history in the last 3 years
- No property destruction over the last 3 years
- Stability with self-injurious behaviors over the last year
- No verbal aggression over the last year
- No VA history towards other clients in the last 3 years
Substance Use Concerns:
- In our experience, we are not the best setting for individuals who are or have a recent history with substance use as we are not a facility or sober living environment. With few exceptions, individuals that do have challenges with substance use have not done well at Grandma's Place because there is such a high level of independence and we do not have 24 hour supervision. Many times individuals end up relapsing, hospitalized or evicted. We will not place someone where their health and safety could be in jeopardy.
